Sorry, it's not meant to be held like that. the knife was designed to be held with your fingers on the grooves and the blade pointed away from your body. you make a punching motion and deliberately miss your target so that the knife cuts on the way by. Google Lucifer/Maze/Knife to see how it is used in a fight. Maze is a character on the show Lucifer who uses Karambit knives. It's a great knife for self defense.

I have pretty fat fingers and my index finger fits in with plenty of room to spare. You should have no problem. I can’t imagine anyone having a finger too fat to fit. The knife is very sharp too. Great deal.
