Q: Any suggestions on brackets to mount this on a forklift?

It has two holes for screws in the bottom. Mount on a flat surface or install a flat piece of metal to mount it to. Mine are mounted to the rollover protection cage.

Q: Does this include wiring harness and a rocker switch?

it has two wires, red and black, approximately 12 inches long for use tieing into the vehicles wiring ... there is no rocker switch with this light
