Excellent book. I love the space for emergency admissions, doctors notes & medications will be so good to keep tract of my health

Exactly as described. Has the perfect amount of space to write in. I take it to all my oncologist appointments. Perfect for labs, symptoms, medical history etc.

I bought this for a friend who has had a number of medical issues recently. The pictures don't really do it justice. The blue and beige "my personal health record keeper" thingy on the front is cardboard and slides off, so that you are left with a plain black, hardcover journal. It is small enough to fit in a handbag, but large enough that there is plenty of space to write. Perhaps most importantly, a large section of the book is dedicated to doctor's appointment visit logs, where one can record reasons for the visit, diagnosis, medications prescribed, test results etc. There is also a small but helpful section to record dental issues. I am so impressed with this book that I am considering buying one for myself.
