Q: Does this include the screw?

No, because the screw would be on the thing you are mouting the camera to. For example, I have two Manfrotto tripods, one Slik tripod, and a helmet mount; they all have the screw, they all came with the screw included. GoPro was the first to go with an other than standard threaded hole in the bottom of the camera.

Q: Can this slide into the Gopro sticker mounts or is it just for attaching to the 1/4-20 thread on camera mounts?

No, the base is a perfect circle, not meant to used with the GoPro mounts. It is strictly for 1/4"-20 threads. As it is, it's the best mount of this type. Solid billet with no nut to pop out and loose, very well constructed. It's worth it. Aloha, bc.

Q: will this work with a joby gorilla pod?

Yes, it works perfectly. I use them together often on my vlogs... it has a straight 1/4,20 thread.

Q: I need the bottom part for this mount to secure itto the top of my full face dive mask. Where can I find the part I need?

Not sure this is the mount you want for that application unless your dive mask has a threaded screw sticking out of it like a tripod.
