Is the widest part of the opening supposed to be perfectly flush?

Its almost flush, but first I had to rub a little bit of olive oil around the outside of the flange and pushed them in firmly. That was the quickest way to get them placed in.

Will these inserts ever come in a 17 or 19mm like the other pump parts you provide? A 21 is still too large

Freemie offers an OEM insert for their collection cups down as small as 15 mm, although price is considerably more than Maymom.

how often should cushions be replaced?

I personally didn’t replace them unless they tore because I washed them after every use

Yes it does. It's a tight fit but you can make it work.
