Jessica I think it would be best if you could measure her waist and then go from there. Does she wear pants or dresses this could make a difference also. My cousin that wears these wears a XL and he weighs about 285 pounds and wears a 3XL pants, if that would help any.

I would most definitely try Amazon brand Solimo Unisex Underwear in M or XL. You don't want them to be loose fitting or they will leak around the inside leg.
