Looks in depth on the concept, causes and effects of errors in particular, and the systemic nature of encouragement vs punishment, trapping, and mitigation in the work culture, and its effects. IMO the author makes a little too strong a case for the pilots, as though it's purely the system to blame for everything but then again I'm no Human Factors researcher. If you can overlook that minor thing there's a lot of useful knowledge here.
