Does it work for Honda Accord low pressure port?

As long as it is R-134A it will work. Some vehicles use R-1234YF and it will not work for those vehicles. As far as I know the civic has never used R-1234YF and has always used R-134A so it will work, but there are some Hondas that do like the Jazz ect. I would recommend double checking that it is R-134A before purchasing... R134A yes--- R1234YF no--- simple answer

Do you have a self seal can tap where the shaft is half the diameter and 3mm longer? I had to mod mine

The simple selection process for a r134a can our can taps work on is if you can go to a store and buy it, our can tap works on that can. If you can only find the brand for sale online, our can tap will probably work for that, but we can't guarantee that because there is no possible way for us to build a can tap that works on all the random sizes these no-name manufactures are using for their cans. I personally went to Wal-Mart, O'rileys, AutoZone, Advanced Auto Parts and Napa and tested it on every R134a can they had and it works without issue. Also when we have had a customer complain that it didn't work, we ask them what brand and it is always a brand that does not sell direct to consumer. Typically a knock off.

Can it be used for R134a tank fittings for a 30 lb car?

If the little cans have the same threads as the big ones then yes., but not sure?

does the tap for self-sealing cans include a check valve / backflow preventer to keep gas from leaking out of hose when removing the tap from the can?

It has a valve for turning it on and off, you just turn it off before removing the connector from the vehicle. Our adapters do that backflow preventer, but can taps do not contain such a device... None do not just ours.
