Q: The concept is great, but I don't have standing ability, so how can I pull my pants/underwear up over my bottom?

You can use it to pull your pants down to the base of your thighs, then grab your pants with your hands and wiggle your butt from side to side until you put them on

Q: Does this practically work for putting on underwear?

It depends on the type of underwear. With some it works better. If it is pants type it will be better.

Q: Does it clip to the back also?

It has clips on all ends I couldn't use mine because it requires 2 hands to use the clips and I only have one working hand. zgood luck
