what is the output line? 1/2 inch pex?

The bottom output is in 3/4 inch - the two top inputs are also in 3/4 inch - the line outputs are all in 1/2 inch. This product is made of PEX material, not metal. And main inlet and outlet adapters are not included. Please add #50141 manabloc adapters and/or V5036.10 manabloc end cap to complete installation.

Can one use a regular pex crimps?

Make sure you get the pex crimp version. This one has the pex tubing attach with Compression fittings that need a special wrench from Viega to be able to easily do them all.

does this include the supply adaptors for the 14 ports?

Those are the outlets and it has more than enough. Just make sure you order the manibloc that has what you want. Some are 1/2 in pex crimps, some are others. Make sure you read the ad to see which one. You will need 2 supply adaptors and a cap or three supply adaptors and no cap depending on how you're using it. Those are separate

can this manifold be installed in a crawl space?

I installed the one that I got in the crawl space under our cabin in an insulated box that we built for our hot water heater. During freezing temperatures when the water lines are not drained the manifold is kept warm by the residual heat given off by the water heater. if the manifold has water in it and the temps reach freezing, the manifold will probably break. Unlike pex plumbing, the manifold is not designed to freeze without breaking.
