If you buy the next size up and trim the insole, won't the cushioning areas hit the foot in the wrong places?

No it will not, if trimmed properly. Having size 5 feet, I normally have to trim, according to the shoes I'm using the insoles for. You just need to place the arch in the appropriate place to see how much to trim from the toe area and possible a little from the heel. Spenco supports are the best that I have ever used. As a ballroom dancer, I have tried a lot.

8-9 is 8-9.5 in Spenco sizes or #3

Can these be used in place of the insole in the shoe?

Well, I have removed the insole of a shoe and used the Spenco instead. Mostly if the original sole and insert combined are too thick. A matter of preference on how much arch support one desires. Your call.

What's the difference between spenco's arch support vs spenco's arch cushion? what is best for flat feet?

The "supports" are made of hard plastic under the insole. The "cushions" are made of dense foam under the insole. I've found the the cushion model much more comfortable but I have high arches and have to have some support in all my shoes. If you have flat feet I wouldn't think you would need much in the way of arch supports. Maybe just the flat insoles would be the most useful . I'd check with a foot specialist for a better opinion than mine.
