can I use it for a spinning wheel lug nut?

I'm not sure what you mean by a spinning wheel lug nut but it may be something similar to the issue I had. I had a locking wheel lug that was so tight it broke every key I tried to remove it with. The lug nut lock did have a spinning ring that prevented anyone from simply using a stripped nut remover. What I did was remove the spinning ring by cutting it off with a drill and some chiseling. Once I got that off I was able use a stripped bolt remover on the remaining lug. I'm only offering this as my solution to the problem I had and not as any guarantee that it would work in your sutuation.

Will this work on spline lug nuts?

It’s designed for smooth nuts and grips well and removes those. Should work on splined lug nuts as well

Will either the 19mm or 21mm extractor work on my 20.25mm diameter locking lug bolt?

I believe it will. The taper on these extractors is pretty significant so I would expect it to work. I have used this set and the smaller socket set to great advantage on totally thrashed bolts.
