You have to keep twisting the bottom part a few times. Some pens come out faster than others. But you will see the gel come through the brush.

Five pens in one box, 4 is whitening gel pen, 1 is desensitising gel, if you are sensitive teeth, you can use it first, 5 minutes later, wash it, then use whitening gel pen.
