Q: Do these vacuum well? I would be putting them in a high traffic area that gets vacuumed at least twice a week.

Yes, I place them in our church at the main hallway where more than 200 people pass by every week. They are easy to clean and vacuum.

Q: Can this be applied on top of carpet and can be removed easily without damage? I live in a apartment (renting) with nasty carpet by the entryway.

As Robert says, this tile over carpet really won't work; the adhesive on these tiles will not hold to the existing carpet surface. Your better solution if you haven't found one by now, would be to go to a remnant carpet outlet, get yourself a single piece of carpet that will cover the entire area and then tack it down around the entire perimeter with a high power staple gun. On the other hand a better solution you might consider--you might want to call your landlord and tell him you want the ratty carpet replaced, given it's in unacceptable condition. Take pictures and complain (in writing) immediately because when you move out, you don't want him saying that YOU ratted up the carpet, giving him an excuse to not return your security deposit. If there are stains in the carpet, who knows, you certainly be concerned that it might be an indication that there mold is growing in the material, definitely a potential HEALTH issue. Could you have possibly seen a bed bug in the carpet? Yes, maybe that's what that was, you can't be sure, but surely the landlord would want to be proactive and CHANGE THE DAMN CARPET. You won't want to have to report a possible bedbug situation, which as a responsible citizen you certainly would have to. You get the idea.
