Q: Does this come with waist and neck set up ?

Waist only it is much to strong for the neck and designed only for the low back.

Q: We are a family with weights of 175 and 250. Can we share the heavy-duty unit by adjusting distance to door?

Yes absolutely. It’s not ideal for the lightest one but if they are well anchored on carpet or a make or if someone holds their hands over their head while in traction insuring they don’t just slide towards the door. Dr F
