What is the difference between hardware v1 and the new hw v2?

The biggest difference I know of is that in v2 you can hook up the traveler wires to either terminal at both ends where with v1, it mattered how the travelers were connected to keep power to the satellite switch. The other physical part I noticed is that v1 had terminal screws for the line/load and the travelers, whereas v2 has wires and needs wire nuts to connect. This adds 3 additional wire nuts in the box, and if your space in the box is limited, this could make a difference. I personally have v1 and am happy with it. Identifying the traveler wires is easy enough as long as they are red and black as they are supposed to be, and I like not having to force the extra wire nuts in behind the switch inside the work box. I have heard that v1 had some hardware and firmware issues, and I did get a defective set the first time around, but the replacement that was sent has worked perfectly. (The first set would not stay connected to wifi and the switch itself got rather warm). Those are the differences I know of.

Is there a True random feature to give a 'lived in look'?

Away Mode lets the KS230 KIT turn your devices off and on randomly to make it look like you’re home, even when you’re away.

Do these work for 12V DC lights? (Asking for RV)

No. AC only. Input Voltage 120VAC 60Hz

Does the satellite work like the pico remotes do on Lutron? Do we need to wire the satellites or are they battery powered?

No matter which version Kasa 3 way dimmer you have a question about. they do not work like Lutron Pico remotes. They are not battery powered so they have to be wired. If you go to Tp-Link Kasa web sight you can find the wiring diagram for the switch.

We installed these dimmers in our renovated home (built in 1905). This was the final 'perfection' to bring modern lighting into a '1905 character home'. We feel the charm of 1900s and touch of modernity with wifi connection and compatibility.......ease of use from anywhere in the world. This is actually the third Kasa switch I’ve bought – the previous two smart dimmable switches installed easily and work great with my Wi-Fi, Kasa app, and Alexa. This 3-way smart dimmer, however, has been extremely frustrating. The wiring is correct and the light itself works from both switches, but the smart part will not connect properly. My phone can see and connect to the temporary “TP-Link_Smart…” network, but when the Kasa app tries to add it to my home Wi-Fi, it just fails over and over. I’ve tried everything: factory resets, 2.4 GHz only, turning off mobile data, standing right next to the router, and repeating the setup many times. Nothing works. What makes it worse is that I’ve already installed it in the wall and used the screws and other hardware, so now I’m not even sure how easy it will be to return. For a brand I usually trust, this specific 3-way dimmer has been a big disappointment and a waste of my time. If you’re expecting a smooth setup like other Kasa switches, be prepared for potential headaches with this one.
