Q: Do these cups leak or are they sealed and the fluids stay in?

The lid is fairly tight, so I haven't had a problem. But I wouldn't leave it sitting upside down if it contained fluid.

Q: are they nested or not?

No. Each one is separate with an attached lid just like the photo show. They came in a large bag. I would imagine if you wanted to take the lids off and nest them you could but then they would not be sterile any longer

Q: Has anyone actually received these with the cups sealed? It seems like many have not, but wondering if anyone has received the item as pictured.

I believe that is because they are sterile. I have not bought in some time but the sterile cups were received as you have pictured.
