A good alternative for De Quervain's (thumb tendon pain) sufferers
This is my third "special" mouse. The first two, a joystick and a vertical mouse, helped me managed carpal tunnel syndrome (my job requires many hours a day on a computer); however, the vertical mouse required a pinch grip which I now fear led to or contributed to the development of De Quervain's, involving the tendons of the thumb - it became excruciating to operate. In desperation I searched the web and came across this mouse from the Netherlands. Despite not being in the 'handshake' position, the angle and curve of the hand on this mouse is sufficient that I am not experience any flare up of carpal tunnel pain/numbness. The trick is to not press down on the wrist or "grip" the mouse, but let the hand lay somewhat loosely, with the weight resting on the flared right side, and let the forearm control the mouse. And the thumb is both supported and resting, so when using it thus far I am pain free. It feels good to have my hand fully supported for a change.Update: After using this a few months now, the pain along my thumb and wrist is significantly reduced -- I'm almost back to normal. Came back to purchase a second one to go with a different computer.**Update: A month in now with this mouse, and it's a definite keeper. My thumb tendon pain has eased considerably overall, and I feel no pain or numbness when using this mouse (upwards of 8 hours a day). I'd still like easier to press buttons though! As the Handshoe website notes, it takes a little getting used to, in the sense that most of us are used to being quite tense when operating mice, something I didn't realize until I had to will myself to stop raising a finger to click or locking my muscles. You can RELAX with this mouse and still retain control. I do recommend getting a friction-free skin for your desktop to reduce drag also. The top is molded all of one piece, is smooth and comfortable, and the mouse seems well made. My only nit is that I would like the option of using less force to press the buttons, they are a bit clicky and old-fashioned in terms of pressure. A note to HIppus to look at the Razr mice for example, which have an extremely smooth button press profile. I do miss having a third button (or a wider 2nd button, so that I can use the third finger to press it), but that is not a deal breaker. The scroll wheel operates very smoothly.I was on the cusp between the small & medium sizes (sizing is on the handshoe website), and got the medium, which fits perfectly.Eventually I plan to try alternating between this mouse & my vertical mouse (and touchpad) - as I think too prolonged use of one mouse (if you work on a computer many hours a day) probably contributes to all the variations of RSI.

Not for me
I had finally had it with the Microsoft wireless mouse and decided to find something different and hopefully more ergonomic friendly. I bought the Hippus Handshoe and enjoyed the usual, quality service from Amazon and its sellers. But as soon as I plugged this mouse in the honeymoon was over for me. I found it to be too large and cumbersome for my tastes. The hand positioning was not natural for me - I had to consciously keep positioning my pinky finger to optimize movement of the device. Moving the mouse so that it stopped precisely where I need it to stop was nigh unto impossible - potentially this might improve with more time on the mouse. So I gave it a good 5 days. One of the nice features of this mouse is that you can adjust the weight of it. On the bottom of the mouse you can unscrew a small closure which house a number of metallic weights; the purpose being to adjust the weights until the mouse acts and reacts as you wish. I did find this feature to be of advantage but for me, even after removing all of the weight I could not operate the mouse in a precise manner. Perhaps it was just me vs this mouse but for me this mouse didn't get the job done. The mouse does not offer/provide the coveted "third button" feature that I and many others have come to expect in a mouse. Moving backwards or forwards was like going back in time. This is a quality built piece of equipment and I have no doubt of that. If it fits your hand and the movement is such that you can use it my guess is that you'll love it. Certainly it's worth a shot.Obtaining approval to return the mouse was very quick as was the refund from the company.

Would have given it 5 stars if I could have avoided surgery
I didn't have to get used to using this mouse. It is a very comfortable and feels natural in my hand. I have been using a traditional mouse for years and started having pain in my right hand and I am left handed. Looking back, I really kept constant pressure on my thumb so that I could grip and maneuver the mouse. I developed trigger thumb as a result. Although it was too late to reverse the damage I caused, I would recommend this mouse to anyone who stays on the computer. I do get some limited relief using this mouse but will continue using it following surgery as it is so comfortable to use.

No more wrist pain!
I'm an application developer so I use my mouse all day. In 2013 I was getting really bad wrist pain at work using a standard mouse and started buying mice that promised to relieve wrist pain. I bought a joystick mouse, a side mouse where your hand is upright, and finally the handshoe. The first 2 ended up causing problems in other parts of my wrist/forearm just making the problem worse. The handshoe mouse actually helped my wrist heal and got rid of my pain!I have been using a medium handshoe mouse at home for 3 years, and a small handshoe mouse at work for the same time. My hand size is average and on the charts it fell in between those sizes. Both work fine, I don't notice a difference when at home vs work.I understand the concern about quality - it seems like a lot of money for what you get, but in my opinion I'm paying for the design and research to make the perfect mouse. One of the mice stopped working after about a year, but once I replaced the USB cord with a cheap one from Amazon, it worked again. Both continue to work after 3 years and many drops to the floor. If you're hesitant, give some of the other cheap ones a try - I gave my old ones to friends having similar issues and the cheaper ones worked for them. Ultimately if those don't work, this is a solid option.

Not for the serious user
I got this because I find every mouse I have ever owned hurts my hand. I tried the G9x the rat 7 and many others. This mouse is huge and bulky, it's bulk restricts your hands and wears your arm out, alk while not even working as good as my $40 g400.It has no dpi settings and seems to be ultra liw dpi. I have to lift it to move my cursor across the entire screen. Not worth the money at all. They even make you pay to return it so I am out $20 for shipping.
